We would ask you to please ensure that the correct form is completed for the student's unit and course of study. Field studies report forms are included in the documentation package mailed to all sites prior to the commencement of field experience. If you have not received a copy of the correct field studies report please contact the Field Experience Office or download the report from the link below.
Interim Field Studies Report
The interim report is to be completed and discussed by the supervising teacher with the preservice teacher at the halfway point of the field experience. The preservice teacher should be given a copy of the interim report. The discussion of the report should provide an honest appraisal of their abilities, indicating areas of strengths and weaknesses. It should also provide the preservice teacher with an opportunity to express any concerns or issues being experienced. If either the supervising teacher or preservice teacher has concerns, these need to be discussed in clear and supportive ways with suggestions being made for improvement. The 'developing' column in the interim report can be used as a means of highlighting areas that need further work.
If the preservice teacher or supervising teacher have serious concerns or if the preservice teacher's professionalism or ethical conduct are at issue, the university liaison academic should be contacted immediately and a At Risk form completed. A copy of this form is found in the Handbook.
Final Field Studies Report
The final Field Studies Report should be completed and discussed by the supervising teacher and preservice teacher on the last day of the field experience. Please note that all objectives must be met at a satisfactory level in order to pass the field experience. One copy should be held at the host site, one copy is given to the preservice teacher and the third copy returned to QUT.

Please note that if required the report may be completed on the relevant web form below. For the final field studies report please complete, print and photocopy (a total of three copies are needed) and ensure each of the three copies has original signatures. One copy should be held at the site, one copy given to the student and one copy returned to QUT.
